What is a Family Status Change? 

A family status change is a qualifying event that allows an employee to make changes to his/her benefits. Family status changes include events such as marriage, divorce, death, birth or adoption of a child and changes in your spouse's employment. 

When may I make changes to my insurance?

You may make changes during Open Enrollment each November (changes are effective Jan. 1). You may also make changes within 30 days of a family status change, per IRS guidelines. 

I have had a family status change. How do I make changes to my insurance?

You must complete the Family Status Change form and provide appropriate documentation.

The chart below gives information for the two most common family status changes: marriage and birth of a baby. If you have had a different family status change — or if you are not sure — contact Kris Akut.

How much will it cost to add another person to my insurance?

View current premium rates here. Please note that your premiums will change in the month of your family status change. Be sure to check your paystub to see that the change has been made. If you have any questions or concerns, contact Kris Akut.

Note: You must turn your completed forms into Human Resources within 30 days of the date of your family status change. If you do not, you will have to wait until the next Open Enrollment in November to make changes which will be effective Jan. 1 of the following year.

Dental   • Fill out change form to add spouse to coverage or to cancel your coverage and go on your spouse's dental insurance. • Fill out change form to add your child to coverage
NOTE: You can wait until the next Open Enrollment to add your baby
