Basic Life Insurance
Basic Life Insurance is provided by MBI at no cost to all full-time employees. Eligible employees are automatically covered for $25,000 in MetLife Life & Accidental Death & Dismemberment Insurance.
Optional Life Insurance
Additionally, employees have the option to buy optional life and/or AD&D insurance in $10,000 increments up to $500,000. No health questions are asked for AD&D insurance or up to $150,000 in optional life insurance. Optional life insurance premiums are based on the employee's age and adjust automatically.
Employees purchasing over $150,000 in optional life insurance and/or $20,000 in spouse coverage are required to complete the Statement of Health Form and return it to Erica Loring, Benefits Administrator, during open enrollment.
Dependent Life Insurance
You may purchase additional life insurance for your spouse and eligible dependent children. Spouse insurance is available in $10,000 increments up to $100,000, with no health questions asked up to $20,000. Premiums for this plan are based on the employee's age. Life insurance for children is available at $10,000 per child at a rate of $0.935 per family, regardless of the number of children. Additional children may be added at no extra charge within 30 days of date of birth.
Changes to Life Insurance can be done during open enrollment in November. To change coverage complete the Life Insurance Change Form and return to Human Resources no later than November 25th.
